Heart to Heart Hospice - - Responsibilities: Provide direct patient care under the RN and Plan of Care; Assist with safe transfers and ambulation per agency policy; Report/communicate changes and document care provided to patient; Practice infection control principles; Maintain confidentiality and professional communication with patient/family and other employees
